The question and answer forms (and guidance notes) formerly used for obtaining information on discrimination and other prohibited conduct under section 138 of the Equality Act 2010 (which were based on the statutory questions and replies set out in the Equality Act 2010 (Obtaining Information) Order 2010) are no longer available online.
Section 138 of the EqA 2010 was repealed on 6 April 2014 and replaced by an informal question and answer procedure. The former questionnaire should only be used in respect of acts or omissions occurring before 6 April 2014.
