A California-compliant medical certification form under the California Family Rights Act (CFRA) for a health care provider to certify the serious health condition of an employee, or the employee's child, spouse, registered domestic partner, parent, parent-in-law (effective January 1, 2022), grandparent, grandchild, sibling, or designated person (as added by A.B. 1041, effective January 1, 2023). This Standard Document is based on the prototype provided by the California Civil Rights Department (CRD) dated January 2023.
