A California-compliant medical certification form under the California Family Rights Act (CFRA) for a healthcare provider to certify the serious health condition of an employee, or the employee's child, spouse or parent. This Standard Document is adapted from the prototype provided by California's Department of Fair Employment and Housing. Integrated notes with important explanations and drafting tips have been added.
