A form that can be completed by an employee to give notice of their intention to take ordinary adoption leave when they have been advised by an adoption agency that they have been matched with a child. This form meets the requirements set out in regulations 16 and 17 of the Paternity and Adoption Leave Regulations 2002 (SI 2002/2788).
