Form T-1

A form completed by an entity appointed as trustee under an indenture governing a company's debt instruments. The form describes the entity's eligibility to act as a trustee under an indenture to be qualified under the Trust Indenture Act of 1939 (TIA). The form requires the trustee to provide certain information about itself and its relationships with the issuer of debt and the underwriters, such as whether the issuer or any underwriter holds any of the trustee's securities and whether the trustee holds any securities of the issuer or any underwriter. The Form T-1 is filed by the issuer as an exhibit to its registration statement when registering a public offering of debt securities.
