A consent form designed for an employee to complete following a request that they attend an examination with an employer's doctor. The form also seeks the employee's consent to the employer approaching the employee's GP or consultant for a medical report and/or medical records. The form may be adapted if the employee's GP or consultant is not being approached for a medical report and/or records.

A consent form designed for an employee to complete following a request that they attend an examination with an employer's doctor.
