A consent form providing an employee or prospective employee's consent to a test for illegal drugs or alcohol. It releases the employer and other parties from liability and discusses testing methods and confidentiality requirements. It should be provided to an employee before administering a drug test. This Standard Document applies only to private workplaces that are not subject to the federal Drug-Free Workplace Act of 1988. It is based on Florida law and is intended for use with employees or businesses located in Florida. Local law may impose additional or different requirements on employers, but this document will be useful and relevant to employers throughout the state.
