Model Qualified Medical Child Support Order

A model qualified medical child support order (QMCSO) for requiring a group health plan to provide health benefits coverage to children of plan participants who are divorced, separated or never married. The plan administrator of a group health plan may provide this model to the parties in a child support proceeding to expedite the QMCSO process.
